About Primary Care

Your primary care physician (PCP) is your partner in health. Beyond just routine care, routine care, treating illness, managing chronic conditions, and serves as your advocate within the healthcare system.

Your primary care physician generally provides you with most of your medical care unless you require specialized treatment for a health problem, at which time he will offer a referral and help coordinate and explain your treatment options.

Your relationship with your PCP should continue throughout your life. At PinnacleHealth, we are committed to the medical home model of expanded care. This model emphasizes a team approach and encourages the patient to become an active partner in his or her health.

Typically, your primary care providers will be identified as family practice, internal medicine or pediatric doctors. In some practices, you may have the option of having your care delivered by a nurse practitioner or a physician's assistant.

Primary care includes:

  • Health and wellness promotion
  • Routine physicals and check ups
  • Health maintenance and disease prevention
  • Counseling
  • Patient education
  • Diagnosis and treatment common illnesses
  • Diagnosis and treatment of acute and chronic illnesses
  • Coordination of care
  • Referrals to specialists
  • Maintenance of health records
  • Medication monitoring/management
