We are delighted to have you as a patient in our practice. We hope to provide quality medical care to you during this time of vital growth in the community and the surrounding areas. We focus on listening to you as a patient. We will work to develop a treatment plan based on standard of care practice and evidence-based medicine.

Our team currently consists of six wonderful staff committed to your primary care needs. We are confident that you will find our group friendly, caring and open toward your needs.

At Carlisle Internal Medicine, we provide full range of services including medical management of diabetes, hypertension, COPD, and their complications. We encourage and teach preventive medicine, vaccinations, annual physicals, preoperative consultation and counseling.

Your First Appointment

You will need to bring the following items to your appointment:

  • Your Insurance Card
  • Photo ID
  • Previous Medical Records
  • List of Your Medications: including the dose and the times you take them; alternatively, you may want to just bring them with you
  • List of Your Concerns: issues you want to discuss with your physician
  • Your Payment: please be prepared to pay your deductibles, co-pays, co-insurance and any other amounts due at the time of your visit

Your Registration Process

For your convenience, the new patient forms are provided below. Please bring the completed forms to your first appointment. If you choose to wait, plan to arrive at least 15 minutes early to complete your registration.

Accepted Insurance

Bring your insurance cards to all of your office visits. Failure to do so or insurances that cannot be verified at the time of service may result in your appointment being rescheduled. Please note, prior to the visit it is the patient’s responsibility to check with their insurance company to verify that Carlisle Internal Medicine is participating with their health plan, that the correct Primary Care Physician/Group in on their card and verify that the insurance is active.

We currently participate with most major insurances.

The physicians at Carlisle Internal Medicine provide the diagnosis and treatment for a wide range of conditions. Services include but are not limited to the following commonly acute treated medical conditions:

  • Cardiovascular diseases such as heart diseases, hypertension, high cholesterol, peripheral vascular disease.
  • Neurological diseases such as Parkinson disease, Stroke, Dementia, restless leg syndrome, headaches.
  • Gastrointestinal diseases such as liver disease, peptic ulcer disease, colitis, rectal bleed.
  • Endocrine diseases such as Diabetes, Thyroid and Parathyroid diseases, osteoporosis.
  • Urological diseases such as Prostate problems, Urinary Tract infections, Over Active Bladder.
  • Musculoskeletal diseases such as arthritis and muscle problems.
  • Pulmonary diseases such as Asthma, COPD, lung infections.

  • Hematological problems such as anemias, clotting disorders, management of blood thinners.

Falls and Senior Adults

A greater percentage of falls each year in the US occur among adults 65 and older. The most common causes of falls in older adults include medication side effects, environmental factors and medical conditions such as Parkinson's disease and arthritis. "Our geriatrics population should ask about a routine fall risk assessment from their healthcare provider especially if they have a history of falls" says Dr. Mary C. Davies. "Most falls in this age group may be prevented by recognizing and modifying these risk factors." Increased mobility and muscle strengthening has been show to decrease fall risk in older adults. Patients are encouraged to research various exercise programs available to them in the community. Let these steps also help maintain your functional independence and well being!

Tips to Avoid Falls in the Home

Home Safety

  • Correct poor lighting/unsafe stairs
  • Remove floor clutter, extension cords, loose rugs or furniture
  • Install grab bars and railings where indicated
  • Promote proper use of mobility devices (canes, crutches, and walkers)

Clinical

  • Routine fall risk assessment by health provider
  • Routine eye exams
  • Evaluate any unexplained dizziness, lightheadedness or weakness
  • Exercise (Thai Chi/balance and strengthening)
  • Make an appointment to see Dr. Davies or Dr. Steele by calling (717) 245-9264 to have your risk assessment. They will also be able to answer any questions you may have during your appointment.
