New Patient Information
Welcome to our practice and thank you for choosing us to serve your health care needs. This website is designed to answer many of the questions that you may have. The more you know about our policies and methods of practice, the better we may be of service to you. Should you have any questions, please do not hesitate to contact us.
If you are a new patient, please bring your insurance card and arrive early to complete any necessary paperwork. If you have been under the care of another physician, please bring your records or have them delivered prior to your appointment.
Billing and Insurance
We accept most major insurance plans including Medicare and Medicaid. Please bring your insurance card with you so that we can make a copy for your medical record. If you have a change in insurance, address or phone number, be sure to notify the receptionist so that the change can be entered into our system.
We accept all major credit cards for payment, such as Visa and Mastercard. All balances are expected to be paid at the time of service.
What to Bring With You
You will need to bring the following items:
- Your insurance card
- Photo ID
- Previous medical records.
- A list of all your medications including the dosage and the time(s) you take them. Alternatively, you may bring them with you.
- List of all your concerns so that you may discuss them with your scheduled physician.
Your Registration Process
Please plan to arrive at least 15 minutes early to complete your registration. You will be asked to sign our HIPAA form, as required by law, so that you are aware of our privacy policies.
Please be prepared to pay your deductibles, co-pays, co-insurance and any other amounts due at the time of your visit.
If you are unable to make your appointment, please let us know at least 24 hours ahead of time so that we may schedule someone else that needs care.
