To obtain copies of your medical records, you must complete a signed authorization form.
You may complete the authorization form by:

To submit the completed form:
- mail to PinnacleHealth Harrisburg Campus, P.O. Box 8700, Harrisburg, PA 17105; or
- fax to (717) 782-3671.
Birth Certificates - PinnacleHealth System does not provide copies of birth certificates. Copies can only be issued by the PA Department of Health, Division of Vital Records. Their website,
