An Accountable Care Arrangement (ACA) is a group of health care providers who provide coordinated care, chronic disease management, and thereby improve the quality of care patients receive. The organization’s payment is tied to achieving health care quality goals and outcomes that result in cost savings.

ACAs coordinate care for a population across multiple medical specialties and care settings, both inpatient and outpatient. It promotes evidence-based care practices and use of information technology.

ACAs are accountable to the patients and the third-party payer for the quality, appropriateness, and efficiency of the health care provided.

Three core principles for the ACA PinnacleHealth has formed with Capital BlueCross:

  1. Provider-led organizations with a strong base of primary care that are collectively accountable for quality and total per capita costs across the full continuum of care for a population of patients
