PinnacleHealth is developing a predictive analytics tool to help create healthcare roadmaps for improved outcomes for selected patients with high burden chronic illnesses. A dedicated team of experts will gather data from throughout the hospital system, as well as clinical input from physicians and staff.
Using the IBM Content and Predictive Analytics technology platform, the team will be able to create a roadmap for patient populations who have certain chronic conditions such as COPD and congestive heart failure. Based on this wealth of data and experience, clinical teams can create appropriate treatment plans for individual patients.
"The goal is to not over treat or under treat," said George Beauregard, DO, senior vice president and chief clinical officer. "We use the overall data to create the predictive model that the clinical team can apply to the individual. That becomes their personalized plan. Ultimately, it’s about turning knowledge into action at the point of care where the benefit to the clinician and patient is greatest.”
This predictive modeling aims to help reduce acute exacerbations of a condition, eliminate Emergency Department visits, and help patients stay home under the care of a primary care physician. Based on past experiences and data, it will provide instructions on specific follow-up visit intervals, treatment recommendations, home care instructions and medication management.
The initial model under construction is targeted towards reducing readmissions related to chronic lung disease, a condition of known high incidence and prevalence. PinnacleHealth anticipates early model results will be available and ready for implementation by the end of May.
