HARRISBURG, PA – The Joint Commission granted PinnacleHealth
Disease Specific Care Certification – Advanced Heart Failure. Only organizations with Get with The Guidelines-Heart Failure bronze level performance or higher are eligible for this advanced certification program. PinnacleHealth has had gold recognition for three consecutive years. Certification was awarded after PinnacleHealth underwent an on-site evaluation and demonstrated compliance with nationally developed standards for heart failure care.
“This Certificate of Distinction recognizes our exceptional efforts to foster better outcomes for heart failure patients. Our latest recognition means that PinnacleHealth continuously provides safe, high-quality care, treatment, and services by identifying opportunities for improvement in our processes and implementing them accordingly,” states Michael A. Young, CEO & President for PinnacleHealth System
Heart failure is a condition in which the heart can no longer pump enough blood to the rest of the body. PinnacleHealth’s Heart Failure Center (HFC) was developed in an effort to provide patients living with heart failure an alternative to frequent hospitalizations. Heart disease is the leading cause of death in the United States.
“We have met expectations of safe, successful transitions of care from the inpatient setting to outpatient settings. This certification is one in a long line of recognitions and achievements for our cardiac staff and programs, and one of which we can all celebrate and be proud,” comments Jean Wiest, vice president Cardiovascular and Pulmonary Services at PinnacleHealth System.
