According to research from PinnacleHealth’s community needs assessment, the Harrisburg population has the greatest number of socioeconomic barriers as measured by income, culture/language, education, insurance, and housing.
Its disparate populations are comprised of uninsured, underinsured, chronically ill, homeless, and low-income seniors who experience barriers to accessing health care services.
Local health centers serve a critical need in providing care to these individuals, but have a number of limitations related to resource capacity, lack of electronic medical records, hours of operation, and availability of primary care and specialty physicians.
PinnacleHealth recognized an opportunity to provide a more integrated and community-wide care coordination approach to serve its culturally diverse and often disenfranchised community.
