Community Needs Assessment
We are committed to assessing community needs and planning community benefit programs for the purpose of improving the health of Dauphin, Cumberland, Perry, Lebanon, and the northern tier of York Counties.
Mission driven healthcare organizations have a long tradition of working to improve community health through community benefit activities.
This dynamic tri-system collaborative has convened to meet the following objectives:
Conduct an objective and comprehensive study of the overall health status of our region.
Include input from individuals who represent the broad interests of the community served by the hospitals as well as those with special knowledge and expertise in public health.
Present consistent findings to our physician providers, health and human service organizations and communities we serve.
Measure and report impact on target populations.
Deliver a useful and valuable report that creates a baseline for strategic planning decisions and can be made widely available to the public.
The results of the community health needs assessment are used to develop strategies designed to improve the effectiveness of community benefit programs, comply with federal tax exemption requirements in the Affordable Care Act, and, most importantly, improve the overall health of the community.
