Coordination of your care can begin in the hospital or at your physician’s office. Your multidisciplinary care team may include:
- Case manager
- Occupational therapist
- Physical therapist
- Physician
- Speech therapist
This team tailors a plan of care appropriate to your condition and communicates regularly to monitor your progress and make adjustments.
What to Expect
Comprehensive care is provided seamlessly from in-hospital to homecare and a variety of conveniently located outpatient settings. Physician follow-up is a key piece of your rehabilitation and assures that you receive the most comprehensive treatment plan available.
Through PinnacleHealth you have access to a variety of specialized programs designed to diagnose and treat conditions resulting from your brain or nerve injury/disorders. In addition to state-of-the-art technology and equipment, our facilities are staffed by professionals who are dedicated to supporting and encouraging you throughout your treatment. All of our staff regularly attend continuing education programs. In addition, many of our staff have earned additional licenses and certifications in their areas of specialty such as MS, hand therapy and lymphedema and neurodevelopmental training.
